Get active, feel better

Our Exercise Referral Scheme helps you become more active in a safe, supportive way if you have a health condition or haven’t exercised regularly.

Who it’s for

The scheme is for people aged 16 and over who:

  • are not regularly active
  • have a long-term health condition
  • have completed any medical rehabilitation programmes

How it works

Step 01

Get referred

Speak to your GP, practice nurse, or health professional about joining the scheme. They’ll complete a referral form and send it to our team. You’ll also receive our contact details.

Step 02

Phone consultation (15-20 minutes)

We’ll call you to:

  • explain the scheme pathway
  • advise discounted prices available with an Active Card
  • book a face-to-face consultation at the chosen Oldham Active centre

Step 03

Face-to-face consultation

Visit your chosen centre for a Face-to-face consultation (30-40 minutes):

  • consultation to gather some more information regarding your health, medical and exercise history.
  • a tour of the facilities
  • Active Card is issued to access discounts
  • Your details are added to our system along with a photo
  • and book suitable dates for a gym induction (if you’re using the gym) and a 3rd appointment for a personal exercise programme.

Payment options with an Active Card:

  • Pay and Play (pay only when you visit)
  • 12-week Ultimate membership for only £60 (includes gym, classes, swim, sauna, steam, and badminton)

Step 04

Face-to-face induction

What to bring to your face-to-face induction:

  • A copy of your prescription or list of current medications
  • Wear suitable gym clothing (changing facilities are available)

Ready to start

Visit your chosen Oldham Active centre:

  • to follow your personalised exercise programme
  • attend gym, class, or pool sessions
  • body composition analysis
  • regular reviews with our experienced team
